Current Status
In a 2003 presentation to the FCC Voip Forum, Jeff Pulver estimated that “less than one tenth of one percent” of US access lines are Voice over IP. XO and COX communications estimate that currently 5% of their voice traffic is VOIP. A study by telephia released in Aug 2006 puts that number to just over 4%, and lists the top 9 Voip providers:

Vonage is clearly ahead of the game. This is not surprising. One of the panelists estimated that Vonage it costs Vonage $200 in marketing to get one subscriber.
I do not have data on penetration numbers for business users compared to home users.
